NEW STUDENTS
New Students for 1st Semester S.Y. 2025-2026
Please prepare a scanned copy or CLEAR photo image of the following documents:
1. CTC - 2nd or 3rd Quarter Grades Certified by either: Principal, Registrar, Class Adviser (A MUST)
2. Photocopy of PSA/NSO BC or Student's valid school ID (A MUST)
3. For SHS applicant - original copy of ESC grantee (Gr 10 completers) can be submitted together with the orig report card later
OLD/CONTINUING STUDENTS
Please sign in. Prepare a scanned copy or clear photocopy of any ONE (1) of the following documentary requirements:
1. Student school ID
2. Latest Grade Slip / or UC Official Receipt of latest payment
3. PSA/BC
RETURNEES
Please sign in. Prepare a scanned copy or clear photocopy of any TWO (2) of the following documents:
1. Student school ID
