for 1st Semester S.Y. 2026-2027
Please prepare a scanned copy or CLEAR photo image of the following documents:
1. CTC - 3rd Quarter Grades Certified by either: Principal, Registrar, Class Adviser (A MUST)
2. Photocopy of PSA Birth Certificate (A MUST)
3. For SHS applicant - original copy of ESC grantee (Gr 10 completers) can be submitted together with the orig report card later
